Pathways to Housing operates the Hartman House and provides the hospitality services for patients in the SCHC HOPE Medical Respite Program. This program is desidned to support the acute medical recovery of individuals experiencing homelessness. While in this program, patients are placed in temporary housing while they receive medical services, case management, and other support through our partners at Shasta Community Health Center.
In March 2023, Pathways to Housing began providing case management and meals to residents of the new South Market Micro Shelter Community which is operated by our partners at United Way of Northern California.
In June 2023, Pathways to Housing began delivering meals to individuals in Shasta Community Health Center's Short-Term Post Hospitalization Program.
We also participate in community education, advocacy, and support as much as possible!

About the Role
Role Summary
The Program Assistant position will provide general support to management and patients that are served in the SCHC HOPE Medical Respite program. This position includes administrative and clerical work, some field work as needed, transporting patients, errands, shopping, delivering meals, and facilitating group activities with patients who need respite/recuperative care and are experiencing homelessness. This position works with a diverse and vulnerable population.
Responsibilities & Essential Functions

1. Develop rapport, communicate effectively, and build relationships with patients who are placed in respite program.
2. Facilitate group activities with patients.
3. Maintain productive relationships with community partners through timely and effective communication.
4. Participate in meetings with collaborating partners as needed.
5. Mitigate any patient issues that arise per protocol.
6. Comply with all policies, procedures, and protocols including safety protocols.
7. Assist management with filing and projects as needed
8. Maintain organization of files and systems.
9. Perform Care Coordinator duties
10. Assist with organization of pantries
11. Assist with receiving and processing donations
12. Perform clerical duties.
13. Assist patients with transportation in a company vehicle as needed.
14. Deliver meals as needed.
15. Shop for supplies as needed.
16. Assist in kitchen with cooking and cleaning as needed
17. Other duties as assigned.

Qualifications
1. Possess a valid California Driver’s license and 5-year acceptable driving record.
2. Possess a valid CPR and First Aid certificate required.
3. Possess a valid Food Handler’s Safety Card required.
