To refer a patient to the SCHC HOPE Medical Respite Program, please fax a completed Referral Form to John Lord, RN at +1 (866) 394-7926.
If you are in need of services, please visit the HOPE Van
or your primary care provider to obtain a referral to the SCHC HOPE Medical Respite Program.
The SCHC HOPE Medical Respite program is a collaboration of Shasta Community Health Center (medical component) and Pathways to Housing (hospitality component).
Medical Respite provides the opportunity to heal, recover, and rest in a safe environment while case managers assist in securing supportive services and housing resources.
A stay at the Hartman House offers short-term residential care for individuals who no longer require hospitalization, but still need to heal from an injury or illness and whose condition would be worsened by an unstable living environment.

The Hartman House opened in February 2022!
Thank you to all of our community partners for helping us achieve this goal! We look forward to serving the community through providing respite care to those in need.
Now that renovations are complete, the SCHC HOPE Medical Respite program is being hosted at the Hartman House while maintaining the previous "scattered site" model, utilizing local motels. This way HOPE can provide medical care, case management, and hospitality services to our patients daily no matter their circumstances!
Location
The Hartman House facility is owned and managed by Pathways to Housing. We have staff on-site 24/7 to maintain a safe, healing, and stable environment.

Visiting Hours are 2 PM to 5 PM
Monday through Friday.
